ABSTRACT

El presente caso clínico se enfoca en el diagnóstico y el tratamiento de erupción forzada en un paciente con trauma dental. Objetivo: Proponer una alternativa de tratamiento multidisciplinario el cual permita incrementar estructura dentaria a través de la extrusión forzada y posteriormente rehabilitarlo en función y estética. Métodos: Se describe el caso de un paciente adulto de 78 años de sexo masculino, sano y sin antecedentes patológicos, acude a la clínica de la Especialidad de Ortodoncia y Ortopedia Dentomaxilo facial, referido por un especialista en Endodoncia, debido a fractura corono-radicular de canino superior derecho, pilar de puente fijo de tres unidades En la exploración intraoral presenta fractura cervical de la corona del canino superior derecho. Como primer paso, se realizó tratamiento endodóntico en la pieza dentaria y colocación de un aditamento intraconducto de soporte (endoposte vaciado), con el objeto de mejorar la tracción ortodóntica. Este aditamento consistió en un poste colado con perforaciones. Se procede a colocar aparatología fija en la arcada superior con técnica MBT (slot 0.022), del molar derecho hasta el canino izquierdo de cementado indirecto y pasivo (con los slots de los brackets alineados). Inmediatamente después, se colocó un arco rectangular 0.019 x 0.025 de acero inoxidable con un doblez de extrusión a nivel del canino superior derecho. En el mismo doblez, se adaptó un loop tipo helix que funcionó como apoyo para colocar la ligadura pasiva (lace back). Resultados : El tratamiento realizado en este paciente es satisfactorio, coadyuvando en su estado de salud general mejorando su autoestima. Conclusión: Aquí se aprovecharon todas las ventajas que ofrece la extrusión ortodóntica forzada, incluso en un paciente adulto mayor, logrando una tracción de cuatro milímetros, que se consiguió gracias al empleo de fuerzas extrusivas ligeras y controladas sobre el órgano dentario afectado. Con la modalidad de tratamiento descrita se puede lograr un alargamiento de corona sin la necesidad de realizar una resección ósea, lo que permite una correcta rehabilitación protésica, devolviendo la función y estética al diente lesionado y brindando un beneficio integral al paciente.


The case of an adult patient with a complicated Crown fracture of the right upper canine due to trauma is reported, diagnosing a class VII Ellis coronal fracture. There are several treatment alternatives that range from the extraction, placement of a bone graft and placement of an implant, to a forced root extrusión with bone removal to allow the biological space and subsequently be restored. In the present case, a multidisciplinary orthodontic forced extrusion treatment is performed, which allows to increase the amount of clinical remnant, preserving the periodontal support and maintaining the biologic thickness, thus achieving to maintain the root remnant with good length so that prosthetic rehabilitation is facilitated. The clinical and radiographic follow-up was 12 months. The multidisciplinary treatment involved: Root canal treatment, forced extrusion with orthodontics, fibrotomy with root planing and fixed prosthesis.

ABSTRACT

Medical literature shows that the co-infection of syphilis and human immunodeficiency virus (HIV) is increasing dramatically worldwide. HIV infection and syphilis have a synergistic relationship. Syphilis increases the risk of HIV transmission and acquisition, while HIV affects the presentation, diagnosis, progression and response to syphilis treatment. The diagnosis of syphilis is made with a non-treponemal reactive test (VDRL or RPR) confirmed with a treponemal test (FTA-ABS or MHA-TP). The opportune diagnosis of neurosyphilis is essential, particularly in the asymptomatic stages, given the high risk of serious sequels and lethality. All patients co-infected with HIV and syphilis with neurological symptoms must be studied with PL and other complementary tests. There is controversy about when to perform a lumbar puncture in co-infected patients who do not have neurological symptoms. However, there is consensus that a CD4 count lower than 350/µl or RPR title greater than 1/32 has indication for the study of cerebrospinal fluid. Therapy with penicillin G in high doses is the treatment of choice, in addition to clinical and serological follow-up that must be done to these patients. (AU)

ABSTRACT

Las dificultades de procesamiento sensorial en niños con Trastorno del Espectro Autista (TEA) están ampliamente descritos en la literatura principalmente mediante el uso de encuestas a padres y observaciones. Con menor frecuencia se han descrito las dificultades de procesamiento sensorial en niños menores de 3 años de edad que han sido posteriormente diagnosticados con TEA. Este estudio retrospectivo está basado en 84 niños (28 diagnosticados con TEA, 28 con retraso en el desarrollo, y 28 niños con desarrollo típico) entre 18 y 36 meses de edad que asistieron a un programa de intervención temprana y cuyos padres completaron el cuestionario Infant Toddler Sensory Profile (Dunn & Daniels, 2002). Los resultados obtenidos en esta encuesta revelan que los niños menores de edad con TEA muestran un patrón específico de procesamiento sensorial en comparación con niños con otros retrasos en el desarrollo.


Sensory processing difficulties of children with Autism Spectrum Disorder (ASD) are extensively described in the literature using parent surveys and observations. The sensory processing difficulties of children under 3 years of age who are later diagnosed with ASD have seldom been described. This retrospective study is based on 84 children (28 diagnosed with ASD, 28 with developmental disabilities, and 28 typically developing children) between 18 and 36 months of age attending an early intervention program and whose parents completed the Infant Toddler Sensory Profile questionnaire (Dunn & Daniels, 2002). The results obtained in this survey reveals that toddlers with ASD exhibit a distinct pattern of sensory processing as compared to children with other developmental delays.

ABSTRACT

La comida chatarra se ha convertido en parte de los hábitos alimentarios de nuestra sociedad, siendo un factor determinante del sobrepeso y obesidad, entre otras enfermedades crónicas no transmisibles, que atentan contra la buena salud que deberían gozar los estudiantes para un óptimo rendimiento académico. Una investigación realizada en Chile a estudiantes mostró que el 58,6 % consume comida chatarra debido a una ingesta inadecuada del desayuno, la omisión de ciertos tiempos de comida; el consumo de alimentos de mala calidad preparados en cafeterías de la misma universidad o lugares aledaños a causa de la falta de tiempo por la distancia hasta sus hogares. La misma realidad se ha observado en nuestra ciudad, de ahí la importancia de realizar y obtener resultados con este estudio. OBJETIVO: Identificar factores y determinantes de consumo de comida chatarra en estudiantes de la Facultad de Medicina, Nutrición, Enfermería y Tecnología Médica de la Universidad Mayor de San Andrés - 2016. METODOLOGÍA: Descriptivo - transversal el análisis estadístico se utilizó SPSS Vr. 19 RESULTADOS: De 100 encuestados 69% son mujeres y 31% varones, el promedio de edad de la población estudiada fue de 22 años, 43% estudian medicina, 58% residentes en La Paz, 59% realiza actividad física, el 28% de las mujeres consume bocaditos, 38% sabe que la comida chatarra causa ECNT y su consumo en La Paz llega al 28% siendo el mayor, 36% consume al menos una vez/semana bebidas gaseosas y el 50,7% del sexo femenino reemplaza el almuerzo por comida chatarra frente al sexo masculino de 48,38%. El consumo de agua es bajo en mujeres, el 32% de 1 a 3 veces/día, el 48% reemplaza el almuerzo por comida chatarra y el 38% de medicina y 28% de nutrición tienen conocimiento de las ECNT que causan la comida chatarra.


Junk food has become part of the eating habits of our society; being a determinant of overweight and obesity are chronic non communicable diseases. According to research students in Chile was observed that 56.8% consumes junk food due to inadequate intake of breakfast, the omission of certain meal times, the consumption of this type of food do in snacks at the university or in surrounding areas because of the lack of time for the trip they make to their homes. OBJECTIVE: To identify factors and determinants of consumption of junk food students of the Faculty of Medicine of the University of San Andres 2016. METHODOLOGY: A descriptive cross-sectional study was conducted using the survey to students of the Faculty of Medicine for the descriptive analysis of data SPSS 19 was used. RESULTS: Sample of 100 students 69% female and 31% are male, The mean age of the study population was 22 years, 43% from medicine, 58% live in the city of La Paz, the total 59% do physical activity, females have a frequency of 28% consumption of snacks, 38% know that junk food causes chronic disease, eating junk reaches 28% in La Paz being the highest, 36% consumes once / week fizzy drinks and 50.7% female replaced by junk food lunch male versus 48.38%. water consumption is low in women of 32% from 1 to 3 times / day, 48% replaces lunch for junk food and 38% medical and 28 % knowledge of nutrition have chronic disease causing junk food.

ABSTRACT

Tuberculosis (TB) remains a major public health challenge. The true incidence of intestinal TB is unknown, as it can be asymptomatic, and by its nature, often diverts its diagnosis to neoplastic diseases or inflammatory bowel disease. Therefore, we must have a high index of suspicion, not only in high risk populations and immunocompromised patients. Diagnostic tests that certify the pathology, don’t always achieve excellent performance. Endoscopic findings are not always clear in differentiating malignancy, and in some cases, a therapeutic trial may be needed to confirm the disease. We present the case of a patient with chronic diarrhea, consumptive syndrome and without respiratory symptoms at its onset.


La tuberculosis (TBC) sigue siendo un reto importante de salud pública. La verdadera incidencia de TBC intestinal es desconocida, ya que puede ser asintomática, y por su naturaleza a menudo desvía su diagnóstico a patologías neoplásicas o de enfermedad inflamatoria intestinal. Por lo tanto, se debe tener un alto índice de sospecha, no sólo en poblaciones de alto riesgo y en pacientes inmunocomprometidos. Las pruebas diagnósticas que certifiquen la patología no siempre se logran ni tienen un excelente rendimiento. Los hallazgos endoscópicos no siempre son claros para diferenciarla de una neoplasia, y en algunos casos una prueba terapéutica puede ser la confirmación de la enfermedad. Presentamos el caso de un paciente con diarrea crónica, cuadro consuntivo y sin síntomas respiratorios al inicio de su cuadro.

ABSTRACT

Hasta 2 013 sólo 130 casos de quistes mesentéricos habían sido descritos en el mundo, por lo que constituye un hallazgo bastante raro de diagnóstico difícil, por la sintomatología poco específica, sin hallazgos imagenológicos patognomónicos, que en todos los casos requiere un abordaje quirúrgico y estudio histopatológico. Presentamos el primer caso descrito en la literatura boliviana, de mesotelioma multiquístico peritoneal benigno en una paciente de sexo femenino, con cuadro de 3 meses de evolución, caracterizado por dolor y aumento de volumen en hipogastrio, asociado a dificultad para miccionar, con evidencia imagenológica de masa pélvica de origen vesical u ovárico, realizándose laparotomía exploradora y estudio histopatológico para determinar diagnóstico, presentó evolución favorable y fue dada de alta con sonda foley. Actualmente realiza controles regulares por ginecología oncológica y oncología clínica.


Until 2 013 only 130 cases of mesenteric cysts have been described in the world, which is a rare find with difficulties to diagnose by uns-pecific symptoms, without pathognomonic imaging findings, which in all cases require surgical approach and histopathological study. We report the first case described in Bolivian literature of bening peritoneal multicystic mesothelioma benign in a female patient with Table 3 months of evolution, characterized by pain and increased volume in lower abdomen, difficulty in urination associated with radiographical evidence bladder pelvic mass or ovarian origin, performing laparotomy and histopathological study to determine diagnosis the patients presented a favorable evolution and she was discharged with foley catheter. Currently she has regular controls by gynecologic oncology and clinical oncology.

ABSTRACT

We report a case of a young patient who was admitted for study and management of severe hypocalcemia. During evolution of the condition the patient presents abdominal pain (biliary colic) related to the presence of Ascaris lumbricoides in the gallbladder, responding to conservative medical therapy.


Reportamos un caso de un paciente joven que ingresó para estudio y manejo de una hipocalcemia grave. Durante su evolución el paciente presentó dolor abdominal (cólico vesicular) relacionado con la presencia de un ejemplar de Ascaris lumbricoides en la vesícula biliar, que respondió a terapia médica conservadora.

ABSTRACT

Con el objetivo de determinar la eficacia de la terapia con Renalof en la urolitiasis cálcica se realizó un estudio observacional, prospectivo, en pacientes con el diagnóstico de urolitiasis cálcica de la consulta del Hospital Julio Trigo, diagnosticados por la presencia de cálculos menores de 2 cm, en vías urinarias por ultrasonografía, TUS y/o TAC abdominal; administrándole Renalof a la dosis de 975 mg diarios hasta 3 meses; se evaluó la presencia de cálculos en cualquier localización, con un análisis descriptivo con determinación de frecuencia. Siendo más frecuente en hombres (86 por ciento), blancos(80 por ciento) y de 30 a 59 años de edad (68 por ciento). Las localizaciones más frecuentes (54 y 43 por ciento) fueron caliciales y piélicos con disminución en el tiempo; la actividad litiásica en el tiempo también disminuyó hasta 92 por ciento, sin efectos adversos importantes. Concluyendo que el Renalof resulta ser un producto eficaz en los pacientes con urolitiasis cálcica idiopática.


In order to determine the efficacy of Renalof therapy in calcium urolithiasis, we conducted a prospective observational study in patients with the diagnosis of calcium urolithiasis from Julio Trigo Hospital, diagnosed by the presence of calculi smaller than 2 cm in the urinary tract by ultrasonography, TUS and/or abdominal CT. We administered Renalof at a dose of 975 mg daily for 3 months, we evaluated the presence of calculi in any location, with a descriptive analysis of frequency determination. Being more frequent in men (86 percent), white (80 percent) and 30 to 59 years (68 percent). The most common locations (54 and 43 percent) were calyceal and pyelic and decreased over time. Also lithiasic activity decreased over time up to 92%, without significant adverse effects. We conclude that Renalof is a product that proves to be effective in patients with idiopathic calcium urolithiasis.

The frequency and severity of human infections associated with Corynebacterium ulcerans appear to be increasing in different countries. Here, we describe the first C. ulcerans strain producing a diphtheria-like toxin isolated from an elderly woman with a fatal pulmonary infection and a history of leg skin ulcers in the Rio de Janeiro metropolitan area.

Se presentan los resultados de una encuesta realizada a los médicos de la Facultad de Medicina de la Universidad de Antioquia, en el segundo semestre de 1997. El proyecto se realizó con el doble propósito de construir conocimiento sobre el tema del climaterio femenino y, en segundo lugar, para aportar elementos pertinentes a la discusión de cambio curricular en que está empeñada la Facultad y la Universidad. Partimos de la hipótesis de que la historia personal del individuo limita las actitudes y condiciona el desarrollo de motivación y destrezas sobre un tema determinado. Se utilizó el censo y las técnicas de encuesta y escala Lickert para indagar sobre variables socioculturales, demográficas y de carácter científico/técnico. Se encontró que el nivel de conocimientos, medido por la nota, fue de 2.85 + 0.83; la actitud frente a la terapia fue baja en el 44 por ciento y alta en el 40 por ciento. Como se había previsto en la hipótesis, la relación entre actitudes y conocimientos fue directa, pero la asociación con variables sociodemográficas no pudo determinarse debido a la homogenidad de la población

Background: There is phenotypic and genetic variability among the species Borrelia burgdorferi that produces Lyme disease. Three gene species and seven serotypes have been defined. Aim: To study the efficacy of two gene species in the serological diagnosis of Borrelia burgdorferi infections in Granada, Spain. Material and methods: One thousand sixty nine sera coming from 1,251 subjects without Lyme borreliosis were analyzed. These subjects were studied for health or pregnancy controls, differential diagnosis of viral disease, diagnosis of syphilis, neurological or rheumatic diseases. In all samples, antibodies against Borrelia burgdorferi (B31 and Pko strains) and against Treponema pallidum were investigated. Screening tests (ELISA and hemagglutination) were followed by confirmations tests for positive samples (Western Blot IgG strain B31 and FTA-abs respectively). A clinical and laboratory follow up was done for subjects with positive serological tests. Results: The global rate of positive antibodies against Borrelia burgdorferi B31 was 8.31 per cent and against the strain Pko was 0.64 per cent. Western blot was negative in 36 per cent of subjects with positive ELISA B31. The distribution of antibodies against the strain B31 was acute herpes virus infection in 16 per cent, gestation in 3 per cent, HIV infection in 6.4 per cent, T pallidum infection in 36 per cent, rheumatic diseases in 25 per cent, neurological diseases in 17.5 per cent and health controls in 7.4 per cent. The percentage of positive Western Blot analyzes were 0.8, 2.1 and 0.4 per cent respectively. A reversion of positive ELISA tests was observed in 6 subjects. Conclusions: The disparity in rates of antibodies against Borrelia burgdorferi in different geographic regions may be due to differences in the serological tests used. The high rate of false positive ELISA tests underscores the need to use other serological tests

ABSTRACT

Some direct methods that can be used for the diagnosis of Lyme borreliosis are the culture, direct visualization or the detection of microbial DNA using polymerase chain reactions, in body tissues or fluids. Unfortunately, all these methods have a low sensitivity. There is a wide assortment of tests and antigens for indirect diagnosis, and the most recommended are ELISA tests and Western blot. The main inconvenient of these tests are the existence of shared serologic reactions, the variability of immune response and the difficult interpretation of results. Therefore, we propose the following guidelines for the diagnosis of Lyme borreliosis: For sero-epidemiological studies and to diagnose infection, antibodies should be determined in subjects with a compatible clinical picture, using an ELISA test that must be positive in at least two separate samples. All positive ELISA results should be confirmed with Western blot analysis, that must be interpreted using established criteria. Polymerase chain reactions should be used when they are available

Background: The diagnosis of cytomegalovirus infections measuring IgG or IgM antibodies has a high rate of false positive or negative results, specially in immunocompromised patients. Aim: To compare the diagnostic yield of antibodies against cytomegalovirus with the measurement of the antigen in peripheral leukocytes. Material and methods: Forty three blood samples coming from pediatric patients with suspected cytomegalovirus infections were analyzed. Low affinity IgG and IgM antibodies against Epstein Barr virus and cytomegalovirus, using indirect ELISA assays, and the virus antigen in peripheral leukocytes, using a commercial immunoperoxidase assay, were measured. Results: Seven patients had positive IgM antibodies against cytomegalovirus. In five of these the viral antigen was detected in peripheral leukocytes. Twenty patients had positive antibodies against Epstein Barr virus, and in 16 patients all serologic tests were negative. Conclusions: There is not a good correlation between antibodies against cytomegalovirus and the detection of its antigen in patients with acute infections

The aim of this work was to relate the avidity of specific IgG antibodies with the time of evolution of human brucella infection. IgG anti-brucella antibodies were measured using an ELISA assay with 8 M urea in 17 patients with recent active brucellosis (group I), 31 patients with chronic active brucellosis (group II) and 120 asymptomatic patients with past brucellosis (group III). Twelve patients of group I (70.6 percent), three of group II (9.7 percent) and none in group III had low avidity antibodies against Brucella. It is concluded that these antibodies have a high diagnostic efficiency for recent Brucella infections

ABSTRACT

Enterococci resistance to antimicrobials has increased lately. We studied the susceptibility to 12 antimicrobials of 150 enterococci strains coming from hospitalized and outpatients, using the agar dilution method. Teicoplanin, followed by imipenem and amoxicilin-clavulanic acid had the lower minimal inhibitory concentrations. No strains of E faecalis was resistant to ampicillin, whereas 14 percent of E faecium had minimal inhibitory concentrations over 8 µg/ml. The high minimal inhibitory concentrations of cefpirone (64µg/ml) renders this antimicrobial useless in the treatment of enterococcal infections. Betalactamase production and resistance to glycopeptides were not detected. Antimicrobial susceptibility of strains coming for hospitalized or outpatients were similar

ABSTRACT

1. The purpose of the present investigation was to study the effect of ß-estradiol crystals implanted in the corticomedial area of the amygadaloid body on the sexual behavior of castrated male rats. 2. The animals were divided into the following groups: group I (N=9), castrated rats; group II (N=4), rats which had been castrated and stereotaxically implanted with cholesterol, both groups being used as controls; group III (N=6), castrated rats con estradiol implants. Latency to the first anogenital exploration, latency to the first mount and mount frquency were determined during the pre-castration and post-castration phases and after the material had been implanted in groups II and III in 10-min observation sessions. 3. There was diminished sexual behavior of the animals in group I without spontaneous recurrence within the period observed. Group II animals, who had undergone implantation of cholesterol, an inert substance, maintained low levels of sexual behavior (post-castration 0.8 ñ 7 vs 0.0 ñ 0.0 and 0.5 ñ 0.5 on the 6th and 9th day afther implantation, repectively). Group III animals presented a gradual increase in the number of mounts (from post-castration 1.2 ñ 0.5 to 6.5 ñ 2.7 and 4.1 ñ 1.0 on the 6th and 9th day after implantation, respectively) and a decrease of mount latency (from post-castration 431.2 ñ 55.9 to 226.1 ñ 119.6 and 51.0 ñ 28.9 on the 6th and 9th day after implantation, respectively) reaching pre-castration levels on the 6th and 9th day after ß-estradiol implantation. 4. We conclude that, under the effect of estradiol, the amygdaloid region can modulate male sexual behavior, thus injdicating a physiological role for estradiol receptors present in this area

ABSTRACT

Se presenta el caso de una paciente con un abscesso hipofisario, el cual fue diagnosticado durante el periodo preoperatorio y drenado por via transesfenoidal con buenos resultados. En la literatura medica se han comunicado 57 casos y, ademas del que se presenta, solamente hay uno en que fue diagnosticado antes de la operacion o la necropsia. La asociacion de manifestaciones de hipofuncion hipofisaria a la imagem radiografica de un tumor sillar y a meningoencefalitis repetida, es muy sugestiva de abscesso hipofisario. El tratamiento de eleccion es el drenaje transesfenoidal
